About Community Healthcaring Kitchener-Waterloo (The Health Centre)

The Health Centre provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.

The Health Centre works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:

Job Title: Registered Nurse; Diabetes Education Program

Salary: $68,770.86 - $ 82,857.14 per year

Permanent, Full-Time FTE 1.0

37.5 hours/week

Job Summary:

The Registered Nurse (RN), Diabetes Education Program provides community-based health assessment, diabetes self-management education, insulin starts and adjustments and behaviour change support for people living with type 2 diabetes in the Kitchener Waterloo region.

The RN, Diabetes Education Program functions within a client-centered health team, in collaboration with CHKW staff, Primary Care Practitioners, Endocrinologists, Community Partners and the Waterloo Community Diabetes program.

The Registered Nurse, Diabetes Education Program participates in the development, delivery and evaluation of programs and services for CHKW and the regional Waterloo Community Diabetes Program.

Responsibilities:
• Provide individual and group education, health counselling and behaviour change support within RN scope of practice (as determined by the College of Nurse of Ontario).
• Implement client centered insulin initiation and/or ongoing diabetes management orders as authorized by referring/primary care practitioner.
• Maintain accurate and up to date clinical notes in accordance with College guidelines and CHKW policy.
• Participate in planning, development, implementation, monitoring and evaluation of Diabetes Education programs for individuals and client populations as determined by CHKW and the Waterloo Community Diabetes Program.
• Participate in quality initiatives as determined by CHKW and the Waterloo Community Diabetes Program.
• As a Student Supervisor, provide orientation, ongoing support, coaching and monitoring in order to meet the requirements of CHKW and of the educational institution.
• As a Volunteer Supervisor, provide support, training, monitoring and on-going communication as needed for the volunteer to successfully fulfill their position description.
• Facilitate the transfer of knowledge across the team to foster growth and development.
• Maintain a valid driver's license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with Community Healthcaring KW.
• Work during both regular and extended hours (evenings/weekends) within Kitchener as determined by the Health Centre. A minimum of one evening per week will be required.
• Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
• Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
• Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
• Work in a manner that preserves privacy and confidentiality.
• Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Centre.
• Work in a manner consistent with the Health Centre's Vision, Mission and Guiding Principles.
• Work in a manner consistent with the Health Centre's policy and commitment to Diversity, Equity and Inclusion in all processes and practices.

Accountability

This position reports to: Manager, Health & Wellness

Qualifications:
• BScN or equivalent combination of education and experience.
• Current registration and good standing with the College of Nurses of Ontario.
• Three years nursing experience, preferably in a community setting.
• Experience with the development and delivery of health education and promotion programs.
• Strong health assessment and clinical skills.
• Experience in diabetes education and care, preferably having achieved and then maintained the Certified Diabetes Educator qualifications.
• Demonstrated experience in insulin management is an asset.
• Experience using evidence-base behaviour change concepts in chronic disease self management.
• Proficiency in the use of a personal computer and software programs including Microsoft Office, email and internet. Experience with electronic medical records is an asset.
• Experience working effectively with culturally and economically diverse populations.
• Familiarity/experience with motivational interviewing principles.
• Ability to take initiative and to work independently.
• Ability to work collaboratively within a diverse team environment.
• Ability to maintain strict confidentiality and act with discretion.
• Ability to communicate effectively in English, both written and verbally. Second language is an asset.
• Ability to maintain strict confidentiality.
• Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
• 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.

Hours:

37.5 Hours per week

Monday/Tuesday/Thursday 9am-5pm

Wednesday 9am - 5pm or 12 pm - 8pm

Fridays 8am - 4pm or 9am - 5pm
